Personalized Plans of Care
Outpatient services are flexible and tailored to individual needs, starting with a complete evaluation to build a care plan that reflects each person’s goals. Using proven, research-based tools like SBIRT—a practical approach that opens the door to early support, to help identify when someone’s alcohol or substance use might put their health or safety at risk. For individuals needing more structured care, the medically assisted treatment (MAT) program offers added support for individuals addressing opioid use, combining trusted medications with counseling. Services also include one-on-one and group counseling, family sessions, education support, and help with vocational planning.
Moving Forward with Support That Lasts
