Centennial Peaks Hospital is a behavioral health facility providing services to both adults and adolescents. Centennial Peaks specializes in medical detox, acute inpatient hospitalization, and intensive outpatient treatment. With psychiatrists, registered nurses, and mental health professionals available 24/7, they are well equipped to treat suicidal ideation, trauma, and acute mental health concerns. Traverse Outpatient Therapy at Centennial Peaks Hospital offers specialized intensive outpatient programs (IOP) and partial hospitalization programs (PHP). Participants can choose the best program with assistance from the treatment team. Most therapy programs include up to 20 sessions. The majority of patients will spend 9 hours per week in group treatment. For patients who want flexibility, day and evening tracks are provided, both virtually and in-person.

Specialty Groups

Centennial has a variety of specialized programs including a woman's survivor group, for women who have been victims of trauma, a young adults track, and an adolescent IOP program. The adolescent group is designed for those struggling with suicidal thoughts or psychiatric symptoms that don't require an inpatient setting.

Aftercare Planning

Planning for discharge is a crucial step in the healing process at Centennial Peaks Hospital. They start working on discharge planning as soon as a patient is admitted to ensure a seamless transition following their stay in the hospital. Discharge planning includes referrals, outpatient counseling, 12-Step support, and more.

Care Options

Levels of Care

Substance use and mental health issues can occur simultaneously or as primary conditions. Effective treatment explores how mental health conditions contribute to addiction, and vice-versa. Many rehabs treat substance use disorders and mental health conditions with personalized care plans.
Detox
Typically the initial stage of treatment for substance use, focusing on medically supervised management of withdrawal symptoms.
Residential
Residential programs for substance use and mental health with structured care and 24/7 monitoring. Treatment takes place on-site, with behavioral health professionals often providing care in group, individual, and family settings.
Sober Living
Not Offered at This Center
Transitional housing designed to support individuals recovering from substance use disorders offering a safe, supportive and structured environment for practicing long-term sobriety, while reintegrating back into daily living.
Day Treatment
In a PHP, patients live at home but follow an intensive schedule of treatment. Most programs require you to be on-site for about 40 hours per week.
Intensive Outpatient Program
An intensive outpatient program (IOP) falls between inpatient care and traditional outpatient services, with therapy and skills-building activities designed to support recovery and provide flexibility. IOP is ideal for those needing support but not the 24/7 care of a residential facility.
Outpatient
Outpatient treatment offers flexible therapeutic and medical care without the need to stay overnight in a hospital or inpatient facility. Outpatient care typically offers a range of therapies and medical interventions individuals can attend alongside daily life.
Virtual & In-Home Care
Not Offered at This Center
The delivery of therapeutic services utilizing technology such as video conferencing, online messaging or phone calls, allowing for flexibility, comfort and increased access to care.
Intensive Inpatient
The highest level of care, medically managed with 24-hour nursing and physician care in a residential or medically-based setting, and can include any combination of medication, counseling and therapy.
Co-Occurring Disorders
Substance use and mental health can occur simultaneously as co-occurring disorders. Treatment for co-occurring disorders involves therapy and other personalized interventions to address both conditions.

I had a horrible experience. I was left in a dirty room that clearly hadn’t been cleaned in days; I would guess months. The staff is way too few people to care for the number of patients. That’s about 1 staff per 6 patients. As a result, there was only one nurse who even treated me with basic human dignity. The rest of the nurses were too busy shuffling paperwork to attend to patients. They even have a secret punishment system called desk program, where you get banned from the few groups that even happen - for an entire day! There’s nothing else to do besides color in children’s coloring sheets using a box of broken crayons. I was intentionally humiliated daily. My experience was pretty common, judging from conversations with other patients. I would not trust these cruel people to care for animals, let alone a human being who needs help.

I know it is very hard for a lot of people to have to be brought somewhere to get the help they need but when I came here I was welcomed with open arms and all of the staff helped so be try much! I made lots of short term friends but it made me feel very safe and comfortable, I 100% recommend Centennial Peaks if you need to be put on a hold. Much appreciation goes out to Maggie for being my biggest support while I was there.

Review from Rehabs.com

My mom has been bipolar since before I was born. We had tried so many things and every time she gets well, she stops her medications. Over and over. When she was in the hospital after an overdose, we thought it was hopeless. She was in the ICU for a week. Then she went to Centennial Peaks Hospital and they put her on a certification. She has been committed many times but nobody ever got her court ordered to stay on her medications before. We really liked the Nurse Practitioner who cared for my mom. He told us a lot about her condition and about how the court system works. A doctor called us a couple times and explained things too. He went to the court to force mom to stay on medication. They helped us get her into treatment with a mental health center where they can monitor and make sure she doesn’t stop her medications. So far it is going pretty good. My mom is angry at the doctor but not at us. It was really nice of them the way they handled it at the hospital and helped us stay on my mother’s good side. I am very grateful for all the work these professionals did to help her not only get stable but to stay stable and safe for the long term.
